Role Overview
We are seeking a Social Worker to deliver community-based services from Ennis Primary Care Centre. The role focuses on early intervention, support, and integrated care.
Location: Ennis Primary Care Centre, Station Road, Ennis
Key Responsibilities
- Provide community-based social work supports
- Work closely with primary care teams
- Deliver client-centred interventions
- Support vulnerable individuals and families
- Participate in multidisciplinary case discussions
Requirements
- Degree in Social Work (CORU registered)
- Experience in primary care or community services desirable
- Ability to manage a varied caseload
- Strong problem-solving and communication skills
- Full driving licence and access to a car is essential

- Full-time role
